Team H.E.A.R.T. is a group of interprofessional students who place patients at the center of care using each of our specialties' expertise to provide them with the best support possible. Using an interdisciplinary approach, we discuss our various perspectives on Alex's case study and how our team dynamic played a role in supporting his well-being and overall health.
