Description
Our group had the opportunity to participate in the Interprofessional Geriatric Educational Practicum, where we worked in interprofessional teams of Pharmacy, Occupational Therapy, and Physicians Assistant students. Teams went into the community and worked with an "Elder Teacher" to conduct home safety assessments, collect a complete medical history, and make recommendations for medication reconciliation. Through this experience, we learned about the importance of interprofessional care for the geriatric population. We concluded that there is a need for OT and pharmacy to work collaboratively to provide comprehensive care for older adults regarding their medication management and routine, health education, and overall well being.
