Description
A collaborative team of students from physical therapy, occupational therapy, osteopathic medicine, and public health developed an interdisciplinary case study centered on J.D., a 70-year-old woman recovering from hip fracture surgery in rural Maine. The project explored the complex interplay of clinical care and social determinants of health, focusing on J.D.'s transition from inpatient rehabilitation to home. Key challenges identified included limited access to healthcare services in rural areas, food insecurity, and anxiety related to navigating insurance and medical expenses. The team proposed integrated interventions aimed at promoting functional recovery, addressing nutritional needs, and supporting mental well-being, while highlighting the importance of coordinated, patient-centered care in underserved communities.
