Description
This presentation reflects on a collaborative case study where students from Dentistry, Osteopathic Medicine, Allopathic Medicine, Social Work, and Dietetics engaged in a multi-disciplinary approach to deliver patient-centered care. Working together to create a wrap-around care plan for our Telehealth pediatric patient, Charlotte Sodo, we navigated the balance between applying our field-specific expertise and integrating empathy, motivational interviewing, and attention to social determinants of health
