Description

Our inter-professional team, composed of public health, medical, and physical therapy students, is collaboratively addressing the social determinants of health (SDOH) through the case of Alex, a patient experiencing substance use disorder, alcohol dependence, homelessness, and unresolved childhood trauma. Our goal is to demonstrate how coordinated, trauma-informed care across disciplines can improve health and functional outcomes for individuals facing complex social and behavioral challenges. Together, we will engage in shared decision-making and coordinated communication to ensure continuity of care and accountability across systems. By aligning our efforts within an inter-professional, patient-centered framework, our team aims to reduce barriers created by SDOH and promote sustainable recovery, functional independence, and overall well-being for Alex.
