Our project shows a student-developed interprofessional case study about a public health patient with chronic pain and comorbidities. Our patient faces many barriers to care, and their condition is exacerbated by social determinants of health. In this project, we display considerations that go into understanding the holistic picture and creating a comprehensive care plan.
