Description

An interprofessional team collaborated to develop a patient case highlighting the impact of social determinants of health on patient care. The case focused on a 67-year-old woman who faced challenges managing chronic urinary tract infections following her transition to Medicare. Using a structured framework to analyze these barriers, students identified and researched potential interventions to mitigate the effects of the identified social determinants of health.
